Facility Audit Trail

Each facility has it's own audit trail. To get to this page, on the navigation bar click Facilities and then select the facility name on the list. Click the Audit trail tab.

The Audit Trail table contains the following columns of information:

  • Date

  • User

  • Description

  • Details

The Audit Trail records entries in the following cases:

  • Edits to the facility details such as:

    • Name

    • Address

    • Designation

    • Phone number

    • NPI

    • State ID

    • PTAN

  • HCPCS contract rate change:

    • Calculation method

    • Percentage

    • Contracted price per unit

    • Up to X loaded miles amount

    • Over X loaded miles amount

  • Mileage contracted changes:

    • Loaded miles

    • Mileage to or from facility

  • Payments:

    • When a payment is posted: The dollar amount is a link to the transaction

    • Claim removals: When a claim is removed from the facility invoice. If available, the invoice number will display. If ti is not available, the invoice date will display.
