Subscribers

Collect non-self subscriber information

When selecting anything other than "Self" as the subscriber of the payer, you can record the following subscriber information:

  • Subscriber first and last name

  • Subscriber date of birth (optional)

  • Subscriber sex (optional)

  • Subscriber address (only need one)

  • Subscriber phone (optional)

Note: The subscriber address can be copied from the residential address.

To view, add, or change subscriber information:

  • Manual claim creation > COB tab: Subscriber fields display when anything but "Self" is entered in the "Subscriber" field.

  • Account page > COB tab: Click the edit icon on the same line as the insurance. From the pop-up window, you can view or edit payer (including subscriber) information. If you want to add a subscriber, click the subscriber field and then select the subscriber from the list. The window will expand to include collection fields for subscriber information.

  • Claim page > COB tab: Click the information icon next to the payer type. From the pop-up window, you can view or edit payer (including subscriber) information. If you want to add a subscriber, click the subscriber field and then select the subscriber from the list. The window will expand to include collection fields for subscriber information.

Subscriber information in the CMS 1500 and Waystar output

Subscriber information will be sent to Waystar including the subscriber's:

  • Address

  • City, state, zip

  • Phone

  • Date of birth

  • Sex

When generating a CMS 1500, subscriber information will display in:

  • Box 4 - subscriber name

  • Box 7 - subscriber address

  • Box 11a: subscriber date of birth and sex if present
